② Combination Type – Skin with melasma accompanied by redness

What is Melasma, and How is it Different from Blemishes?

Melasma is more than just a phenomenon where excessive melanin pigment is produced and discolors the skin.

It is characterized by the excessive activation of pigment cells called melanocytes, which are located in the deeper layers of the skin.

Melanin produced by activated melanocytes can spread over a large area, sometimes showing a repetitive pattern.

Consequently, it can be described as a complex pigment disorder that affects the dermal layer and remains in the skin long-term.

In particular, external stimuli such as UV exposure and hormonal changes are major causes that further activate melanocytes.

Additionally, subtle inflammatory responses and vascular factors within the skin can also act in combination.

Unlike simple moles, a typical characteristic of melasma is its ill-defined borders and the way pigment spreads over a large area.

Although the number of melanocytes may be observed within the normal range in tissue biopsy results,

a clear increase in the size of these cells and their dendritic processes (protruding structures) can be confirmed.

This results in melanin being transferred more widely and efficiently to surrounding skin cells.

Some patients may also exhibit patterns where melasma lesions are accompanied by subtle capillary dilation or flushing reactions.

In such cases, it may be separately classified and described as 'vascular melasma'.

Melasma is less a disease in itself, but rather a phenomenon where the skin reacts sensitively to external stimuli and

appears due to various irritations in a structurally unstable state.

Therefore, management that goes beyond simply removing pigment, but also improves the underlying causes and skin structure, can be considered important.

While dual toning may be necessary for general pigmentary disorders, it can be considered even more important than simple toning, especially in cases of melasma.
