Subcision (New Skin Needle) Procedure: Our Approach
3-Step Process of the New Skin Needle Procedure
Step 1: Separating superficial skin adhesions (Using 27G and 25G needles)
Broad fibrous adhesions formed in the superficial skin layer can be precisely broken using micro-needles.
This process delicately removes the fibrous structures that pull scars downwards.
Through this, we expect to alleviate noticeable depressed areas of the skin.
Step 2: Removing deep adhesions (Using a 25G cannula)
Adhesion tissues firmly embedded in the deep dermal layer
using a cannula, can be gently separated over a broad area.
Compared to the Nokor needle used in the past (which was large and had a high likelihood of bruising and pain), this method is gentler and reduces the burden of recovery.
This method can be applied to various types of scars, such as broad rolling scars or boxcar scars.
This step focuses on minimizing the burden of bruising and pain.
Step 3: Inducing tissue regeneration (Using micro-needles)
Finally, micro-needles are used to induce micro-bleeding in the skin tissue.
This creates conditions where blood flow and growth factors can concentrate.
Such an environment can stimulate fibroblasts to promote collagen production.
Importance of Combination Therapy
To maximize the positive effects of the procedure and aid in faster recovery, we consider a combination treatment approach.
① Concomitant PDRN Injection (DNA Injection Procedure)
During the New Skin Needle procedure, PDRN injections are also applied.
This can promote wound healing and help reduce the burden of post-procedure redness and hyperpigmentation.
As an auxiliary method that supports the regenerative capacity of scar tissue, it is included in the basic composition of the procedure, not as an optional addition.
② Combination with Er-YAG Fraxel (Considering Recovery Period)
The Subcision procedure primarily focuses on improving adhesive structures within the skin.
However, there may be limitations in managing skin surface texture or fine irregularities.
Therefore, we may consider applying Er-YAG Fraxel together after the procedure.
This procedure acts on the superficial skin layer and can help improve skin texture, pores, and subtle depressions.
This is expected to reduce the burden of the recovery period and have a positive impact on skin improvement.

PDRN and Fraxel Combination Therapy as a Standard
Combination therapy, which can enhance the recovery process and procedural efficiency,
is considered a fundamental part of the procedure, not an option.
Accurate Diagnosis and Procedure Recommendations Tailored to Each Individual
Subcision treatment is not suitable for all types of scars.
For scars that are deemed unnecessary to treat, we frankly do not recommend the procedure.
As medical professionals, we believe it's important to accurately assess the patient's condition before recommending any procedure.
What is Saesalchim (Subcision)?
Scars have a characteristic of being pulled downwards by the skin underneath.
They appear as indented forms on the surface, with their cause lying in the lower dermal layer.
This is due to a structure where fibrous tethers adhere to and pull the scar area downwards.
For the skin to return to its original position, a process of breaking these adhered tethers is necessary.
The procedure that separates these adhered fibrous tissues is called Saesalchim (Subcision).
Principle of Saesalchim (Subcision) Treatment
1. Separation of Adhered Tissue (Release)
The fibrous tissue adhered from the inside of the skin
is separated using a precise needle (25~27G) or cannula (25G).
This releases the tension that was pulling the skin downwards.
The depressed scar area can naturally rise and improve.
2. Inducing Micro-Bleeding (Regeneration Signal)
The micro-bleeding that occurs during the process of separating fibrous tissue
serves as an important signal to promote new collagen production.
This activates fibroblast activity,
creating an environment where new tissue can be generated in the scar area.
3. Improving Blood Flow and Promoting Tissue Regeneration
If necessary, microneedling treatment and PDRN (DNA injection) are applied together to
induce blood circulation and concentration of regenerative cells to the treated area,
thereby aiming to simultaneously activate wound healing and collagen synthesis.
When can Saesalchim (Subcision) Treatment be Considered?
When applying Duoderm immediately after removal
Please replace it only when there is enough fluid (exudate) and it starts to peel off. Keeping Duoderm on for at least 7 days without removing it can aid in recovery.
For the overall management period, we recommend about 10 days.
If Duoderm use is difficult or an allergic reaction occurs, you should consistently apply a regenerative cream or hydrating gel to prevent the area from drying out.
It is important to diligently apply sunscreen with SPF 50 or higher and avoid touching the wound area.
If Duoderm was not applied after removal
In this case, you can carry on with your daily life while focusing on moisturizing care and consistently using sunscreen.
