Histopathologically, they do not penetrate the dermis; they are characterized by excessive melanin deposition within the epidermis,

with relatively clear boundaries observed between the lesion and surrounding normal tissue.

This characteristic makes them highly responsive to epidermal-targeting laser treatments, such as Q-switched 532nm lasers,

and distinguishes them from melasma or mixed pigmented lesions that affect the dermis.

 

532nm Wavelength

The 532nm wavelength is a green light wavelength with one of the highest absorption rates by melanin pigment, allowing it to selectively target pigmented lesions confined to the epidermis.

532nm lasers are typically used in a Q-switched manner, delivering high-energy pulses in ultrashort durations of mere nanoseconds to instantly destroy only the pigment particles.

These short pulses are selectively absorbed by melanin, converting into thermal energy while simultaneously generating a photomechanical effect, which breaks down melanin particles.

Some of the fragmented pigment then sheds off as scabs, while others are expelled through the lymphatic system by macrophages.

This removal of destroyed pigment results in the visible lightening or disappearance of lentigines.
